Job Title: Bilingual Registered Nurse – Home Visiting Nurse

Job Summary:
The Bilingual Family Connects Home Visiting Nurse delivers in-home health services to families with newborns throughout the Tulsa. Using a structured, evidence-based approach, the nurse conducts comprehensive assessments of maternal and infant health, provides guidance and support, and connects families to appropriate community resources. This role is essential in fostering early parent-child bonding, promoting family wellbeing, and ensuring that each family receives timely support across twelve domains of physical and psychosocial health.
Primary Responsibilities:
Conduct home visits following the model and document assessments and interventions in real time using an electronic health record system.
Perform health and physical assessments for postpartum mothers and newborns to ensure safety and wellbeing.
Evaluate family needs in areas such as maternal mental health, infant care, parenting support, housing, financial stability, substance use, and domestic violence.
Establish trust with families to collaboratively develop individualized family support plans.
Make appropriate community referrals, follow up on services, and assist families with scheduling and attending appointments.
Educate families on maternal and child health, positive parenting, early childhood development, and preventive care practices.
Maintain a caseload and meet weekly visit targets as defined by the Director of Nursing.
Submit accurate and timely documentation within 48 working hours of each visit.
Attend and participate in clinical supervision, team meetings, and staff development sessions.
Promote and uphold agency values of compassion, respect, learning, adaptability, and achievement.
Support agency performance and quality improvement initiatives.
Perform other duties as assigned.
Required Qualifications:
Current RN license in the state of Oklahoma.
Associate, Bachelor’s, or advanced degree in Nursing (A.S., B.S.N., or higher). If A.S. degree, at least one year of relevant nursing experience is required.
Bilingual (English and Spanish)
Valid driver’s license and reliable transportation for home visits.
CPR certification (must be maintained).
Experience working with diverse populations and an understanding of cultural competence.
Strong organizational and time-management skills with the ability to multitask.
Proficiency in Microsoft Office Suite; ability to quickly learn new software and technology.
Ability to lift or move up to 50 pounds.
Willingness to work a flexible schedule to meet family needs.
