Patient Coordinator – Summary of Responsibilities
Are you looking for a TEMPORARY PART-TIME position? This is a great opportunity for individuals who enjoy community work and are not needing long-term employment. Serve as a liaison between patients and healthcare providers, connecting individuals to community resources and addressing non-clinical needs to support whole-person care.
Key Responsibilities:
Build trusting relationships with patients and families to offer guidance and support
Assist patients in accessing food, housing, transportation, and social services
Collaborate with healthcare teams to identify and address non-clinical needs
Maintain accurate documentation and conduct timely follow-ups
Communicate effectively with patients, care teams, and community partners
Support scheduling and administrative duties as needed
Qualifications:
High school diploma required
2+ years in healthcare or social services
Familiarity with local community resources and services
Proficient in Google Suite and strong communication skills
Ability to handle sensitive data with confidentiality and empathy
Strong organizational and time management skills
Bilingual or culturally sensitive experience is a plus
