Nurse Practitioner / Physician Assistant (Post-Acute Care)

Full-Time  

Salary $120,000 – $140,000 per year


About the Role

We are seeking a compassionate and skilled Nurse Practitioner (FNP/AGNP) or Physician Assistant (PA) to join our dedicated post-acute care team. You’ll play a key role in improving outcomes for patients transitioning from hospital to skilled nursing or long-term acute care settings.

Responsibilities

•    Conduct comprehensive patient assessments and review medical histories

•    Order, interpret, and follow up on diagnostic tests (labs, imaging, etc.)

•    Diagnose and manage acute, chronic, and complex medical conditions

•    Develop and implement individualized treatment plans, including medication management

•    Collaborate closely with attending physicians, rehab specialists, and nursing staff

•    Educate patients and families on diagnoses, medications, and preventive care

•    Maintain accurate and timely documentation in our EMR (RXNT)

•    Advocate for health promotion and disease prevention within post-acute environments


What We’re Looking For

•    Proven experience as a Nurse Practitioner or Physician Assistant in post-acute, hospitalist, or rehab settings

•    Skilled in managing multi-morbid and complex patient populations in SNFs or LTACs

•    Knowledge of healthcare regulations and evidence-based patient care standards

•    Excellent communicator and team collaborator in an interdisciplinary care model

•    Patient-centered professional with adaptability and a commitment to lifelong learning

•    Active and unrestricted FNP, AGNP, or PA license (Oklahoma preferred or compact eligible)
