Position: On-Call Substitute Caregiver


Schedule: Generally 24 hour shift 7 am – 7 am


Overview:  The substitute caregiver provides a visible presence on the premises and assist residents with the daily living skills. The substitute caregiver is responsible for distributing medications to the residents and ensuring that medication are being taken as prescribed by the physician. The substitute caregiver is responsible for maintaining the house rules, conducting monthly fire drills, and maintaining the property in presentable, safe and sanitary condition.


Responsibilities:


Observes and monitors residents and documents changes in behaviors and mental and medical conditions.

Informs lead staff persons, case managers and other pertinent parties of residents’ needs or

concerns.

Participates in preparation and organization of meals; promotes social, dietary and hygiene skills.

Reports dietary concerns to appropriate parties.

Monitors and assists residents in learning daily living skills. Transports clients as requested.

Monitors safety and security of the treatment environment.

Assists or provides leadership in management of behavioral emergencies. Assists with maintaining cleanliness and safety of facility.

Keeps up-to-date documentation and audits residents charts as directed, answers phones, enters information into computer, and schedules appointments.

Provides timely, accurate documentation that meets or exceeds billing criteria and uses appropriate psychosocial terminology. Must adhere to Oregon Administrative Rules (OAR) and standards of care.

Assists residents with banking, shopping, cooking, cleaning, laundry, mental & physical health provider appointments, and social service providers.

Follows planned activity schedule and activity programming.

Administers medications when permitted by law and as required by unit; keeps mandatory documentation pertaining to medication.



Qualifications:


High school diploma or equivalent and a general understanding of the population served. Training provided.
