Essential Job Duties:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.   • Assists residents with dressing, grooming, eating, bathing, positioning, turning, toileting, and exercising.

  •       2.Follows care plans established for each resident, encouraging their efforts and participation.

  • Recognizes and reports observations and problems to the Charge Nurse immediately (i.e. change in resident condition, complaints from resident or family, behavioral problems including interventions attempted and the results, or inability to complete tasks).

  • Notifies RNM of any changes that might need to take place with care plan.

  • Keeps privileged information about residents confidential.

  • Accepts direction from supervisors.

  • Transfers, transports and escorts residents as needed.

  • Answers call lights promptly.

  • Attends In-service training and education sessions as assigned.

  • Works effectively with all other staff in team-oriented working relationship with other nursing staff and other departments.

Other Duties:

 

  1. Maintains safe and sanitary work station and equipment

 

  • Assist with orientation of new staff or as a preceptor for NA students.

  • Attend inservice training and education sessions as assigned.

  • Performs specific work duties and responsibilities as assigned by supervisor.

 

Physical Requirements/Working Conditions:

 

  1. Frequent standing, stooping, bending, stretching, squatting; may be exposed to blood and body fluids which may contain HIV and/or HBV; must be able to transport residents via wheelchair, geri chair or shower chair, pushing up to 350 pounds unassisted.  Must be able to lift 35 pounds transfer, turn and position a resident weighing up to 125 pounds unassisted, or with the assistance of one for a resident weighing over 125 pounds.  Must be able to stoop, bend, stretch, squat, stand, and walk for up to 90% of the work day.  May be subjected to offensive odors and combative behavior.

 

Education and Experience Requirements:

 

  1. Must have an active, unencumbered CNA license in the State of Oregon or

  2. Must meet the requirements to be an unencumbered CNA licensed in Oregon (may work up to four months after completing NA Class or moving in from out of state while pursuing certification through OSBN).

  3. Current BLS CPR certification preferred.

 

 

Other Requirements:

 

  1. Ability to understand and follow instructions in English and communicate effectively.

 

  1. Ability to document care given.
